Assisted living emerged from nursing homes in 1981 as a new social experiment that promised a more enriched and higher quality life for its fragile, elderly residents. But the extraordinary healthcare demands of this disadvantaged population were addressed only superficially via assistance with ADLs. The underlying multitude of chronic diseases that ravaged the bodies of the residents and that created the need for assistance with ADLs were not addressed by this model. 

The physical separation of residents from their offsite physicians impeded timely response to changes in condition. This was not only because the mobility of many residents was impaired that made outside travel exceedingly difficult, but also because many residents had cognitive challenges and thus required caregiver accompaniment to physician offices. Furthermore, communication and coordination between the resident and their families, the assisted living community, and healthcare providers was usually incomplete and often inaccurate.

A model of Integrated Care overcomes these shortcomings. Blending healthcare with the social needs of residents allows assisted living communities to provide the most comprehensive approach to resident care that has proven to achieve the best outcomes.

This presentation describes the rationale for and value of Integrated Care and the specific outcomes that have been demonstrated by CMS (The Center for Medicare and Medicaid Services) as well as in a large national assisted living corporation.

At the conclusion of this educational activity, participants should be able to: 

1. Describe the healthcare that was provided in early models of assisted living.
2. Articulate the 3 components of Integrated Care.
3. Describe why assisted living is NOT considered a post-acute provider.
4. State the most common reason that cause residents to come and to leave assisted living.

Dr. Steven Fuller, PhD, DO, MS

Dr. Steven Fuller is a triple board certified Pulmonary and Critical Care specialist for more than 20 years.  During this time he was also the medical director of skilled nursing facilities, home health and hospice companies, and a long term acute care hospital.  For the past 10 years he has focused exclusively on improving healthcare delivery to the assisted living setting. His efforts have included developing a healthcare coordination service, a House Calls company, a healthcare analytics service for assisted living, and a service to reduce assisted living readmissions.  Dr. Steven Fuller has served as the Corporate Medical Director for a large nonprofit senior housing corporation, and been the Executive Vice President for the largest geriatric medical practice in the US southeast who delivers care exclusively to assisted living residents.
