Description:
Transitioning from the hospital to post-acute care is a high-risk period for medication-related adverse events. Medication discrepancies are common during care transitions and can result in adverse drug events that potentially impact resident outcomes and subsequent health care utilization. In this session, pharmacist Bethany Bramwell will review medication risk management strategies and emerging transitional care practices aimed at reducing risks associated with transitions to and from the long-term care setting.

Speaker Biography
:
Bethany Bramwell is a Clinical Consultant Pharmacist with a passion for geriatric healthcare. Currently, she serves as the Senior Director of Customer Experience & Growth for Guardian Pharmacy of Missouri & Kansas, overseeing business development and clinical services for partner long-term care centers across the Heartland. She graduated from Southwestern Oklahoma State University’s School of Pharmacy in 1999 and is Board Certified in Geriatrics. Mrs. Bramwell is a member of the American Society of Consultant Pharmacists, has received Medication Therapeutic Plan Authority from the Missouri State Board of Pharmacy, and has advanced training in Dementia Care and Antimicrobial Stewardship.


Continuing Education:
