ICD-10-CM in the Long Term Care, Swing Bed and Skilled Nursing Setting

The goal of this training is to provide ICD-10-CM training through lecture, discussion and coding exercises, including the need to follow ICD-10-CM Coding and Long Term Care Guidelines and to understand the impact of ICD-10-CM codes with the implementation of PDPM.

Target Audience: Health care professionals who assign or who use ICD-10-CM Codes in the Long Term Care, Swing Bed or Skilled Nursing setting and who will need to understand the relationship between ICD-10 and PDPM. This includes but is not limited to health information (medical record) professionals or designees, Directors of Nursing, MDS Coordinators, Swing Bed Coordinators, LTC Supervisors, billing personnel, Registered Nurses, Licensed Practical Nurses, Advanced Registered Nurse Practitioners or any other personnel interested in learning the basics of ICD-10-CM Coding, LTC Coding Guidelines, Patient Driven Payment Model (PDPM).

Note: While PDPM will apply only to skilled admissions starting October 1, 2019, information covered in this workshop will also be appropriate for non-skilled admissions as the importance of accurate code assignment, sequencing and quality of documentation is the same no matter what setting or payment status.

What to Bring: Each participant will need to bring a 2019 ICD-10-CM Code Book.

Presenter: 

Barbara S. Michaelis, RHIA. An independent Health Information Management Consultant specializing in the training on ICD10-CM in the LTC Long Term Care setting, with 45+ years of experience in the Health Information field in both the acute and long term care setting. She was an AHIMA ICD-10 approved trainer from 2012 through 2016 and was member of the KHIMA ICD-10 Task Force in preparation for and at the time of ICD-10 implementation.

Objectives:

Day 1:
- Understand how the ICD-10-CM code book is organized.
- Accurately assign ICD-10-CM diagnoses codes.
- Understand, interpret and apply ICD-10 coding guidelines for accurate code assignment.
- Understand the basics of PDPM and the relationship between ICD-10 Code and PDPM.
- Practice applying coding guidelines and assigning accurate codes through coding exercises.

Day 2:
- Understand, interpret and apply Long Term Care Coding Guidelines.
- Understand the relationship between ICD-10-CM, PDPM and the MDS.
- Recognize how provider documentation affects codes assigned and how lack of documentation will affect PDPM, and need for documentation improvement.
- Understand the difference between the admitting diagnosis, primary diagnosis and principal diagnosis and the importance of accurate sequencing of diagnoses.
- Create a plan to work with providers on documentation improvement.

- Coding Guidelines.
- Discussion of chapters in code book and completion of coding exercises.
- Relationship of ICD-10-Codes and Patient Driven Payment Model (PDPM)

Day 2

ICD-10-CM Code assignment in the Long Term Care, Swingbed and Skilled Nursing Settings.
Long Term Care Coding Guidelines and coding exercises
- Relationship between ICD-10-CM, PDPM and MDS.
- Documentation needed from providers and potential need for documentation improvement.
- Difference between admitting, primary and principal diagnoses and the importance of accurate sequencing of diagnoses.
- How to create a plan to work with providers on documentation improvement.
- Coding Concerns: Individuals are encouraged to submit diagnoses and/or situations which they would like discussed. Examples, scenarios may be submitted, but do not include and protected health information.

Continuing Education:

This educational event has been approved for up to 16.0 continuing education hours in the core of Administration for Licensed Adult Care Home Administrators, Adult Care Home Operators and Dietitians by the Kansas Department of Aging and Disability Services. Approved provider number (LTS A0009).

LeadingAge Kansas is approved as a provider of continuing nursing education by the Kansas State Board of Nursing. This course offering is approved for 16.0 contact hours applicable for APRN, RN, LPN or LMHT relicensure. Kansas State Board of Nursing provider number: LT0102-1116.
